If you are unhappy about any aspect of how we process your data you have a number of options available to you.
Firstly, we would request that you notify us directly. You can send an email to email@example.com and we will respond to you as soon as possible. Alternatively you can send a letter to our clinics, details of which can be found here:
Please provide your name and address, details of how to contact you, and as much information about the complaint as you have. We will investigate the matter fully and respond to you within a reasonable timeframe. We will also look to fix whatever the issue is where possible.
If you do not want to bring the issue to us first, you can raise the matter with the Data Protection Commissioner.
